Trevelyan Court

Trevelyan court is an extra care scheme in Newcastle upon Tyne. There are 40 apartments/flats within this community for which we provide independent assisted living for people over 55.

Our on site care assistants deliver care calls to residents through out the day are also on site for night support as all flats also have a care alarm service.

Much the same as domiciliary care services in Newcastle once we receive a referral or enquiry for a potential resident at Trevelyan Court we meet with the individual to understand their needs and preferences. The care and support needed can then be tailored towards them and established before moving in.

Peterborough

What to expect once you have decided to appoint Merit Homecare to support with care that addresses your individual needs and allows you to remain at home.

Our Care Plan Manager will visit you and your family, the purpose of the visit is to discuss your health and social care requirements. This plan will be based on your likes and dislikes and will form a “person centered care plan”. The information within the plan will tell us all about you as a person, what level of support you require and how the carers are to deal with various tasks such as personal care, medication, nutritional needs, social inclusion preferences and any other topics that are important to you. The care plan manager will draw up a schedule of care that will inform the care support staff when they need to visit, how long the visits will be and what they will be doing when they are there. For the duration of your time with Merit Homecare we will continue to visit you on a regular basis to monitor the quality of our service and to make any adjustments to your support plan.

To support our care staff:
